Reminder: Complete your 2022 annual COIC disclosure

Annual requirement for Conflicts of Interest and Commitment (COIC) Disclosure

This message is a reminder to complete your annual COIC disclosure (for 2022) now.  You should also update the COIC disclosure form throughout the year as necessary.

Per the Conflicts of Interest and Commitment (COIC) Policy at http://policy.iastate.edu/policy/conflict/ and the COIC Procedures, Applications, and Guidance Document at https://www.provost.iastate.edu/sites/default/files/wdclientcss/Faculty/Policies/PAG-COIC.pdf

All Faculty, P&S staff, Contract Staff, Graduate Assistants and Postdoctoral Scholars are required to disclose whether or not they believe they have a conflict.  Failure to disclose may result in sanctions or disciplinary actions in accordance with the Iowa State University misconduct policies and procedures.

The COIC disclosure is accomplished through an electronic process available at https://login.iastate.edu through ‘COIC’.   The COIC Disclosure site contains a screening question and then a “Graduate Assistant Form”, “Short Form” or “Standard Form” for your disclosure based upon your position responsibilities and your answer to the screening question.

Instructions for COIC Disclosure

1. CLICK HERE FOR LINK TO DISCLOSURE FORM. Otherwise, you can sign into the Iowa State Okta home page at https://login.iastate.edu, click the ‘COIC’ button, then on the COIC tab, and click ‘COIC Disclosure Form’.

2. Answer with your appointment type and click ‘Next’.

3. Answer the questions to which you are directed.  If asked to provide information about one of your external activities, please give enough detail that reviewers not familiar with your situation can understand.  Avoid abbreviations when possible. You may navigate through the pages by clicking ‘Next’ or ‘Previous.’ Alternatively, there is a dropdown function at the top of the page.

4. If you have more than one entity to disclose on any given question, complete the information for one entity and then click ‘Add another Entity’ at the bottom of the page to repeat the section.

5. When you have completed and electronically signed the disclosure, click on the “Submit” button to forward the form to your supervisor for review.

NEW:  If you submitted a 2021 COIC Disclosure and would like to pull in last year’s answers into your 2022 Disclosure, follow the “Copy and Amend” instructions below.

1. Sign into the Iowa State Okta home page at https://login.iastate.edu and click the ‘COIC’ button.

2. Click the “xForms” tab, scroll to the bottom of the “xForms” screen, and click on “All My xForms.”

3. Click the file folder icon next to your 2021 COIC Disclosure, then click ‘Ok’ when asked “Copy for Amendment?”

4. Review each question and answer and, if necessary, update your answers.

5. When you have completed and electronically signed the disclosure, click on the “Submit” button to forward the form to your supervisor for review.
